Important Notice from IROSM: Neck injections are not right for everyone. After an exam at IROSM in Hialeah, Dr. Michael Erdman will tell you whether an injection fits your neck, which type, or whether another option, including a surgical opinion, is the better choice.

Types of Neck Injections

Facet Joint Injection

Medicine is placed into or next to a small joint at the back of the neck. Used for a deep, aching neck pain from joint wear.

Medial Branch Block

Numbing medicine is placed on the tiny nerves that carry pain from a facet joint. If the pain eases, it confirms the joint is the source.

Cervical Epidural

A steroid injection near an irritated nerve root in the neck. Used for pain, numbness or tingling that travels down the arm.

Radiofrequency Ablation

For facet joint pain confirmed by test blocks, heat is used to quiet the small nerves that carry pain from the joint.

Trigger point injections may also be used for tight, painful knots in the neck and shoulder muscles.

Which Injection Fits Your Pain?

The right injection depends on where the pain starts:

  • Pain mostly in the neck, worse when looking up or turning, often points to the facet joints.
  • Pain, numbness or tingling down the arm often points to a pinched nerve. See pinched nerve treatment in Hialeah.
  • Tender knots in the muscles between the neck and shoulder point to muscle pain.

Shoulder pain can also come from the shoulder itself, such as the rotator cuff. Dr. Erdman checks both. Learn why neck pain spreads to the shoulder and arm.

What to Expect

Before

Dr. Erdman reviews your exam and imaging. Tell him about any blood thinners, diabetes or allergies.

During

The skin is cleaned and numbed. Using live X-ray or ultrasound, he guides a thin needle to the target and injects the medicine. The injection itself usually takes a few minutes.

After

Your neck may feel sore or numb for a short time. Most people go home the same day. The steroid can take several days to start working. Dr. Erdman will follow up to see how much it helped and plan next steps.

Possible side effects include soreness, a short rise in blood sugar in people with diabetes, and, rarely, infection, bleeding or nerve irritation. Dr. Erdman will review the risks with you.

What the Research Says About Neck Injections

  • Radiofrequency ablation for facet joint pain: In a study of people with long-lasting neck joint pain after whiplash, confirmed with test blocks, patients were randomly given radiofrequency ablation or a fake (sham) procedure. Pain stayed down much longer after the real procedure: a median of 263 days compared with 8 days. Lord SM, et al. Percutaneous radio-frequency neurotomy for chronic cervical zygapophyseal-joint pain. N Engl J Med. 1996;335(23):1721-1726. View the study.
  • Epidurals, therapy or both for arm pain: A study randomly split people with neck and arm nerve pain into epidural steroid injections, therapy and medicine, or both. At 1 month the main result was similar across groups. At 3 months, more people in the combined group had a good result. Cohen SP, et al. Epidural steroid injections, conservative treatment, or combination treatment for cervical radicular pain: a multicenter, randomized, comparative-effectiveness study.
