Hip osteoarthritis is complex, but its hallmarks include an unhealthy environment for cartilage that leads to cartilage loss, bony spurs (osteophytes), and narrowing of the hip joint space. When the lining of the joint (the synovium) becomes inflamed, it causes hip swelling. As the joint narrows and the hip bones push together, bone bruises (bone marrow lesions) may result. Together, these processes can produce chronic hip pain for patients with this condition.
The treatment gap is when a patient has failed all conservative non-surgical treatments and continues to live with pain and limitation, but is not ready for surgery. This is extremely common in osteoarthritis, and two out of three surgeons believe there is a treatment gap in early osteoarthritis.
