Orthobiologics

What is Orthobiologics?

Orthobiologics refers to a field of regenerative medicine that focuses on using biological substances derived from the patient’s own body or other natural sources to promote healing and improve outcomes in orthopedic conditions. In this article, we delve into the world of orthobiologics, exploring its principles, applications, and potential benefits. In the Institute of regenerative orthopedics & sports medicine, we only use autologous preparations.

Understanding Orthobiologics

Orthobiologics encompasses a range of biological substances that are used in orthopedic treatments. These substances can be classified into several categories:

  1. Platelet-Rich Plasma (PRP): PRP is a concentrated solution of platelets derived from the patient’s blood. Platelets contain growth factors and other bioactive molecules that can enhance tissue repair and regeneration.
  2. Bone Marrow Aspirate Concentrate (BMAC): BMAC is obtained by extracting a small amount of bone marrow from the patient’s pelvis or another suitable site. It contains a rich concentration of mesenchymal stem cells and growth factors that aid in tissue healing and regeneration.
  3. Microfragment Adipose Transfer: MFAT injections, also known as Micro-Fragmented Adipose Tissue injections, are a type of orthobiologic treatment that harnesses the regenerative potential of adipose (fat) tissue. This innovative approach utilizes the patient’s own fat cells to promote tissue healing and regeneration in various medical conditions
  4. Amniotic Fluid and Membrane Products: These products are derived from donated amniotic tissue and contain growth factors, cytokines, and extracellular matrix components that can support tissue repair and reduce inflammation.
  5. Exosomes: Exosomes are small extracellular vesicles that play a crucial role in intercellular communication. In addition ,  these vesicles are released by various types of cells, including stem cells, immune cells, and other somatic cells. Furthermore, exosomes are involved in the transfer of biological molecules such as proteins, lipids, and nucleic acids between cells, influencing cellular functions and playing a role in various physiological and pathological processes. In conclusion, exosomes are still consider illegal to use in the USA as up 2023. In addition, patients can enroll in clinical trial to safety use exosomes.

The Applications of Orthobiologics

Orthobiologics has gained recognition as a valuable tool in various orthopedic applications, offering potential benefits in:

1. Sports Medicine

Athletes often face musculoskeletal injuries that require swift and effective treatment. Orthobiologics, such as PRP and BMAC, can be used to accelerate healing and promote tissue regeneration in conditions like tendonitis, ligament sprains, and muscle strains. Moreoever, these therapies have the potential to shorten recovery times and enable athletes to return to their sports sooner.

2. Osteoarthritis Treatment

Osteoarthritis, a degenerative joint disease, causes pain and stiffness, limiting mobility and diminishing quality of life. Orthobiologics, particularly PRP, BMAC,MFAT  have shown promising results in alleviating symptoms and promoting cartilage repair in osteoarthritic joints. Also, by stimulating the body’s natural healing processes, these treatments aim to improve joint function and reduce pain. Lastly, regenerative medicine and orthobiologics have given us another options for those patients in the treatment gap. Finally watch the video below to see if you are in the treatment gap.

3. Fracture Healing

Orthobiologics can play a significant role in supporting the healing of fractures. By providing a concentrated source of growth factors and stem cells, BMAC can enhance bone regeneration and accelerate the healing process. This can be especially beneficial in complex fractures or cases where the natural healing response is compromised. In addition, Dr. Jorge Gonzalez has been able to treat difficult to heal fractures with orthobiologics. Finally, please watch video below to see how we can treat these types of fracture.

4. Spinal Conditions

Certain orthobiologic treatments can be utilized in the management of spinal conditions, such as degenerative disc disease and disc herniation. Injections of PRP or Bone marrow stem cells into the affected area can help reduce inflammation, stimulate tissue repair, and potentially alleviate pain, offering a non-surgical alternative for some patients.

Potential Benefits and Considerations

Orthobiologics present several potential benefits for patients:

  • Minimally invasive: Many orthobiologic treatments are performed through injections, avoiding the need for extensive surgery and reducing associated risks.
  • Autologous sourcing: Using the patient’s own biological materials minimizes the risk of adverse reactions or immune responses.
  • Enhanced healing and tissue regeneration: Orthobiologics stimulate the body’s natural healing processes, potentially leading to improved outcomes and reduced recovery times.

However, it is important to consider certain factors:

  • Research and evidence: While orthobiologics show promise, it’s essential to acknowledge that the field is still evolving. Further research and clinical studies are needed to establish the efficacy and long-term effects of these treatments in different orthopedic conditions.
  • Treatment selection: Orthobiologic therapies are not a one-size-fits-all solution. The choice of treatment depends on various factors, including the patient’s condition, the severity of the injury or disease, and the physician’s expertise. But a thorough evaluation and consultation with a qualified orthopedic specialist are crucial to determine the most appropriate course of action.
  • Cost considerations: Orthobiologic treatments can vary in cost, and insurance coverage may differ depending on the specific procedure and location. It’s important for patients to discuss the financial aspects with their healthcare provider and understand the potential expenses involved.
  • Regulatory guidelines: As with any medical intervention, orthobiologic treatments are subject to regulatory oversight. Different regions may have specific guidelines and regulations regarding the sourcing, processing, and administration of these biological substances. Lastly, compliance with these regulations ensures patient safety and treatment quality.

Conclusion

Orthobiologics offer a promising approach to orthopedic care, utilizing the body’s natural healing mechanisms to promote tissue repair and regeneration. From sports medicine to osteoarthritis and fracture healing, these treatments have the potential to revolutionize the field of orthopedics.

While orthobiologics provide exciting possibilities, it’s important to approach them with a balanced perspective. In addition, patients should have a comprehensive understanding of the available treatments, their potential benefits, and the considerations involved. Also, consulting with a knowledgeable orthopedic specialist is vital to determine the most suitable course of action based on individual circumstances.

As research continues and technology advances, the field of orthobiologics will likely expand, providing even more innovative solutions for orthopedic conditions. Furthermore, by harnessing the power of the body’s own healing abilities, orthobiologics pave the way for a future where regenerative medicine plays a crucial role in optimizing patient outcomes and improving quality of life.
