How carpal tunnel syndrome is checked, from Dr. Jorge A. Gonzalez, MD, triple board-certified

Carpal tunnel syndrome happens when the median nerve is squeezed as it passes through a narrow tunnel at the wrist. It can cause numbness, tingling and weakness in the thumb, index, middle and part of the ring finger.

In this guide, Dr. Jorge Gonzalez, MD at IROSM in Fort Lauderdale explains the warning signs, simple checks you can do at home, and the tests used in the office to confirm carpal tunnel. In the video, he shows how to check for carpal tunnel at home.

Signs That Point to Carpal Tunnel

  • Numbness or tingling in the thumb, index and middle fingers
  • Symptoms that wake you at night, and shaking out the hand to get relief
  • Tingling when driving, holding a phone or reading
  • Dropping things or trouble with buttons and small objects
  • Weakness or a flattened area at the base of the thumb in later stages

Simple Checks You Can Do at Home

These checks can hint at carpal tunnel, but they cannot confirm it. Stop if they cause pain.

Wrist Bend Check

Hold the backs of your hands together with the wrists bent and fingers pointing down. Tingling in the thumb side of the hand within about a minute can be a sign. Doctors call this Phalen’s test.

Tap Check

Gently tap the middle of the inside of your wrist. A tingling or electric feeling into the fingers can be a sign. Doctors call this Tinel’s sign.

If these checks bring on your symptoms, or your hand is numb or weak, have it checked. Carpal tunnel that goes on for a long time can lead to lasting weakness.

How Carpal Tunnel Is Tested in the Office

1. History and exam

Dr. Gonzalez asks where and when you feel symptoms, then checks feeling, strength and the thumb muscles, and rules out the neck and elbow as the source.

2. Nerve ultrasound

An ultrasound scan in the office shows the median nerve at the wrist. A swollen nerve supports the diagnosis. The scan can also show a cyst, extra muscle or other cause of pressure. It is quick and does not use needles or radiation.

3. Nerve tests

In some cases, a nerve conduction study may be ordered. Small sensors measure how fast signals travel along the nerve. This helps show how severe the pressure on the nerve is.

What Happens After a Diagnosis

Treatment depends on how severe carpal tunnel is. Mild cases often start with a night wrist splint and activity changes. A guided injection may help some people. When the nerve is under more pressure, a release procedure may be the better fit.

At IROSM in Fort Lauderdale, Dr. Gonzalez offers ultrasound-guided carpal tunnel release through a very small opening. See carpal tunnel treatment in Fort Lauderdale and compare carpal tunnel surgery vs non-surgical options. IROSM also treats trigger finger and thumb arthritis.

What the Research Says About Carpal Tunnel Testing

  • Ultrasound of the median nerve: An evidence-based guideline reviewed the studies on ultrasound for carpal tunnel. It found that measuring the size of the median nerve at the wrist with ultrasound is accurate and may be offered as a test for carpal tunnel. It also found that ultrasound probably adds value to nerve tests and can show other causes of pressure at the wrist. Cartwright MS, et al.
