An intraOsseous injection is when the physician is able to put a needle or a drill inside the bone to inject. In addition, we have been able to perform Intra-Osseous(IO) injection with cement as well as with PRP and stem cells. Patient presented with a medial tibial plateau fracture that had not healed after 4 months of non weight bearing. Unfortunately the patient had a history of osteoporosis and had a physical job as a Register Nures(RN) in the emergency department that she was unable to do due to her fracture. After discussing surgery for the non healing fracture, she decided to try Intraosseous (IO) injection of growth factors derived from her own blood. Special thanks and shout out to Dr. Steve Sampson and Dr. Luna for first telling me about the procedural technique. 6 weeks after the procedure, repeat MRI of the knee showed complete resolution of the fracture line and patient was pain free. Furthermore, after a couple of weeks of physical therapy to strengthen the muscles of the knee and leg, patient is now back walking and back in the emergency room treating and saving lives.
Treatment gap is when a patient has failed all conservative non-surgical treatments , continues with limitations and pain but are not ready for surgery. This is extremely common when it comes to osteoarthritis. Subsequently two out of three surgeon believes that we have a treatment gap in early Osteoarthritis.
